THE SOUL GARAGE

We are moving to a paperless system for Soul Garage events. Once you fill out a form for your student(s), you will have access to update it from year to year, rather than have to fill out a new form each time.

The MEDICAL RELEASE FORM only needs to be filled out once per year, unless you need to make changes or updates. Each school year, we will ask you to review the form you previously submitted and verify that the information is still correct.

The PERMISSION FORM can be filled out once for the whole year. If additional events are added that are not listed on the form, you can simply edit the form you previously filled out and add that new event, rather than fill out a new form.
